Summer Health Office Hours

The health office is closed for the summer.  A nurse will be in the office once a week to process all paperwork received.  If you need to speak to a nurse please email one of us and we will call you when we are in the office next we are in the office. 

Sports Paperwork

  • All appointments for the summer physicals with the school doctor are filled at this time. You will need to submit a physical from your own doctor.

    Required in order to be cleared to participate:

    1. Physical completed by your doctor on the state-mandated 
         Please make sure all forms are filled out completely, they are signed by the doctor and have the doctor's stamp. 
    2.  for all 9th and 11th graders. 
    3.  All athletes need to be registered on the RSchool Registration Site
    4. If your child has asthma, allergies, or diabetes a health plan will need to be submitted. All forms are listed below:

No student will be allowed to weight train or participate in summer practices without a new physical or a health history update form submitted to the nurse by June 18th.  Late forms will be processed periodically during the summer as the nurse’s hours are limited.

If you have an appointment already Please note that no student will be examined without the entire medical history portion of the physical form filled out and signed by a parent/guardian prior to the exam.
